Community Resouces

This work incentive only applies to individuals whose earnings are below an annual income threshold, as established by Social Security.
Unity of Indiana Family and Individual Handbook
The Unity of Indiana Family and Individual Handbook provides important information about the services and supports available through Unity of Indiana. It outlines the rights, roles, and responsibilities of individuals receiving services, as well as those of family members, guardians, and staff. The handbook serves as a guide to help individuals understand their choices, advocate for their needs, participate in person-centered planning, and ensure services are delivered with dignity, respect, and accountability.

Social Security Benefit Coverage
Social Security Disability (SSD) provides monthly financial benefits to individuals who are unable to work due to a qualifying physical or mental disability that is expected to last at least 12 months or result in death. The program helps replace lost income and may also provide access to healthcare benefits for eligible individuals. Eligibility is based on medical criteria and, for some programs, work history and contributions to Social Security.
Guardianship Papers
Guardianship Papers are legal documents used to establish a guardian for an individual who is unable to make certain decisions independently due to age, disability, or incapacity. These documents outline the guardian's authority and responsibilities for making decisions related to the person's health, safety, finances, and overall well-being. Guardianship is granted through a court process and is intended to protect the rights and best interests of the individual.
